Four Pillars of Healthcare in Grand County

In 2010, the Network completed a Qualitative Evaluation with focus groups and one-on-one interviews. The results indentified community strengths and challenges. 

In September, 2010, 43 community members worked together to create short- and long-term strategies based off goal areas. The 2010 Qualitative Evaluation and Strategic Plan results fall into four categories: 

  1. enhancing the safety-net infrastructure;
  2. implementing healthcare system navigation and linkages;
  3. educating the community of health literacy and medical homes; and
  4. implementing a health information technology system for healthcare providers and human service oganizations. 
These are the four pillars upon which Grand County's integrated healthcare delivery system will stand.  Since that time, the Network has worked to assure all programs and services fall within at least one of these four pillars.
