NEW! Behavioral Health Navigator Program

Grand County Rural Health Network, Mind Springs Health, Middle Park Medical Center and Grand County Government began collaboration on the Behavioral Health Cooperative of Grand County in 2016 through a grant to develop a county wide assistance program focusing on mental and behavioral health access for the residents of Grand County.  The Behavioral Health Cooperative of Grand County was instituted with a goal to improve access to care; including transportation needs, education and empowering patients and their families. The Behavioral Health Cooperative of Grand County practices innovative methods of care delivery and care transition, including the utilization of Behavioral Health Navigators who can provide brief interventions, serve as a mental health coach and remove treatment barriers.
